New patients can choose one of two ways to check in:
ONLINE: Complete & Send (Valid email Required)
Steps:
1) Go to our Patient Check-In page and launch the respective form by clicking ‘online’ under the appropriate Form tab at top,
2) Complete electronically on your computer and follow the prompts,
3) Check your email to approve and send to DECCT.
DOWNLOAD: Complete, Scan & Upload
Steps:
1) Download the forms from our Patient Check-In page by clicking ‘download’ under the appropriate Form tab at top,
2) Print out, complete by hand, scan and save to your computer,
3) Upload your forms.
