    Using the tools on this page, you can securely send your information or medical records directly to our office via an encrypted form. You can also complete patient check-in forms electronically, update insurance information, or update your records with our file upload tool.

    Patient Check-in

    If you are a new patient, and haven’t yet checked in, first download and complete the Patient Check-In forms. Be sure to save your completed check-in forms to your computer for any future updates.

    Outside Lab Results, Imaging, & Reports

    Upload Lab Results/Reports from Other Providers:

    Using the Patient Information Upload form, you can send us lab results, imaging, or medical records from another medical office (.docx, .pdf files accepted).  If you have a hardcopy, simply scan to a pdf file and attach it to your form.

    Insurance Changes

    Revise Insurance Information: To upload new information or changes to your existing plan, complete the Insurance Change Form.

    Update Existing Medical/Demographic Records

    To update your existing medical or demographic records electronically,  follow the instructions below to use the Patient Information Update Form and upload your updated information.

      Updating Your Existing Patient Check-In Information:

      Adobe Acrobat Reader Required

      If you originally completed the Patient Check-In Forms by hand,

      1)  Edit changes to your original saved forms OR ‘Download’ the respective forms again, print out and make your changes,

      2)  Scan & Save to your computer (.PDF, .doc, or .txt file format),

      3)  Compete all Upload form fields below and select the revised file to upload,

      4)  Click Submit.
